How to Request a Contribution

To request a contribution from Fidelity Bank, an organization must be:

  • An IRS designated 501(c)(3) nonprofit
  • Active in a community we serve

A requesting organization must complete a grant request and submit the form along
with the following information:

  • Your organization’s most recent annual income statement and balance sheet
  • Budget for your project
  • One-page description of your organization’s purpose
  • List of your Board of Directors and officers
  • Copy of your IRS tax status determination letter
