Accounts can be opened at any Fidelity Bank location.

Information needed to apply for an account

 

Details required by all owners to open an account:

  • Name and address of business
  • Name and address of person opening account
  • Business Employer Identification Number (or business owner's SSN)
  • Copy of valid Acceptable Form of Identification for owner of business

Additional documents required at account opening:

  • Letter from IRS certifying Employer Identification Number (EIN)
  • Fictitious name statement (Oklahoma)
  • Certification of sole proprietorship status (Oklahoma)

Details required by all owners and controlling person to open an account:

  • Name and address of business
  • Business Employer Identification Number (EIN)
  • Name and title of person opening account
  • Personal information for all business owners and controlling person
  • Tax ID (for U.S. persons), residential address, copy of valid Acceptable Form of Identification
  • Percentage of ownership for each beneficial owner and controlling person

Additional documents required at account opening:

  • Letter from IRS certifying Employer Identification Number (EIN)
  • Partnership agreement showing business name and name(s) of partner(s)
  • Current business name filing document
  • Certified copy of Limited Partnership or Qualifications
  • Meeting minutes signed by all partners authorizing signatory

Details required by all owners to open an account:

  • Name and address of business
  • Business Employer Identification Number (EIN)
  • Personal information for all business owners and controlling person
  • Name and title of person opening account
  • Tax ID (for U.S. persons), residential address, and a copy of valid Acceptable Form of Identification
  • Percentage of ownership for each beneficial owner and controlling person

Additional documents required at account opening:

  • Letter from IRS certifying Employer Identification Number (EIN)
  • Articles of Incorporation or certified copy of state certified corporate charter
  • Operating agreement or bi-laws of the corporation
  • Signed minutes from the board of directors designating signers

Details required by all owners to open an account:

  • Name and address of business

  • Personal information for all business owners and controlling person
  • Name and title of person opening account
  • Tax ID (for U.S. persons), residential address, and a copy of valid Acceptable Form of Identification
  • Percentage of ownership for each beneficial owner and controlling person

  • Articles of Organization or certified copy of other state charter
  • Operating agreement

Details required by all owners to open an account:

  • Name and address of association/organization
  • Tax ID of association/organization
  • Personal information about the controlling person
  • Name and title of person opening account
  • Tax ID (for U.S. persons), residential address, copy of valid Acceptable Form of Identification
  • Percentage of ownership for controlling person

Additional documents required at account opening:

  • Letter from IRS certifying Employer Identification Number (EIN)
  • Association/organization charter
  • Signed meeting minutes authorizing the account signers and transaction authority
